Job Title:
Certified Nursing Assistant (CNA) / Home Health Aide (HHA) — PACE Program (Multiple Locations in San Diego County)
About the Role:
We are recruiting Certified Nursing Assistants (CNAs) with Home Health Aide (HHA) certification to support Participants in a PACE (Program of All-Inclusive Care for the Elderly) environment. This role provides direct care and support services to older adults in accordance with PACE standards, care plans, and applicable state and federal regulations.
PACE is a community-based model of care designed to support seniors safely and with dignity. Team members work collaboratively with interdisciplinary care teams in a highly structured and regulated care environment.
Primary Responsibilities:

Deliver personal care services as outlined in participant care plans, including assistance with activities of daily living (ADLs)

Support participant mobility, hygiene, grooming, and basic daily needs

Monitor and document observations in accordance with PACE policies and regulatory requirements

Comply with all HIPAA, infection control, and safety standards

Communicate changes in participant condition to clinical team members appropriately

Participate in required trainings and quality standards dictated by the PACE program
Required Qualifications:

Valid and current CNA certification

Current HHA certification
-
Ability to pass all pre-employment requirements (including background, health screening, and credential verification)

Demonstrated ability to work within compliance-driven healthcare environments

Strong communication skills and ability to work with interdisciplinary teams

Reliability, professionalism, and a compassionate approach to care
Preferred Qualifications:

Previous experience with elderly populations

Experience in PACE, home health, assisted living, or community care settings

Bilingual (Spanish/English) a plus, depending on location

Positions available across multiple PACE centers in San Diego County

Scheduling and worksite assignment are determined by program needs and availability
