We are seeking an experienced Home Health Scheduler / Staffing Coordinator to join our growing non-skilled home care agency. This position is responsible for managing daily caregiver schedules, ensuring client visits are staffed appropriately, and maintaining efficient scheduling operations in a fast-paced home care environment.
The ideal candidate has previous experience scheduling caregivers in a home health or home care setting and understands the importance of continuity of care, caregiver availability, client needs, Medicaid requirements, service authorizations, and geographic scheduling.
This role requires a highly organized, detail-oriented, and proactive professional who can anticipate staffing challenges, solve problems quickly, and maintain accurate schedules while delivering excellent service to clients and caregivers.
Key ResponsibilitiesHome Care Scheduling Operations
- Manage daily caregiver schedules, assignments, and coverage needs
- Coordinate caregiver assignments based on client needs, availability, qualifications, and geographic location
- Monitor open shifts and proactively secure coverage
- Handle call-outs, schedule changes, cancellations, and urgent staffing needs
- Maintain continuity of care by matching caregivers consistently with clients
- Ensure schedules are accurate, updated, and communicated timely
Medicaid & Home Care Compliance
- Understand Medicaid home care scheduling requirements and service authorizations
- Ensure scheduled hours align with approved client service plans
- Maintain awareness of scheduling requirements related to EVV and payer expectations
- Monitor missed visits, schedule changes, and documentation requirements
- Support compliance with agency policies and applicable state guidelines
Workflow & Process Management
- Follow established scheduling workflows and operational processes
- Maintain accurate caregiver availability, time-off requests, and scheduling records
- Identify scheduling issues and resolve problems before they impact client care
- Communicate scheduling concerns and trends to leadership
- Assist with improving scheduling efficiency and operational workflows
Geographic Scheduling Management
- Optimize caregiver schedules based on client locations and service areas
- Understand travel time and geographic considerations when assigning caregivers
- Create efficient schedules that support caregiver retention and reliable client coverage
- Identify opportunities to improve route efficiency and staffing coverage
Communication & Team Collaboration
- Serve as the primary scheduling contact for caregivers and internal teams
- Communicate professionally and effectively with caregivers regarding assignments and expectations
- Collaborate with the Clinical Supervisor regarding client needs and caregiver concerns
- Partner with leadership to address staffing challenges and operational needs

Qualifications
- Minimum of 2β3 years of scheduling experience in home health, home care, personal care services, or a related healthcare environment
- Experience scheduling CNAs, caregivers, or personal care attendants preferred
- Knowledge of Medicaid home care operations preferred
- Understanding of EVV systems and scheduling software preferred
- Strong understanding of caregiver staffing workflows and client service coordination
- Excellent communication, problem-solving, and organizational skills
Preferred Experience
- Experience with home care scheduling platforms such as AxisCare, HHAeXchange, WellSky, or similar systems
- Experience managing schedules for Medicaid-funded clients
- Experience working in a high-volume home care environment
