Position Summary
The Coordinated Entry Services Specialist plays a vital role in connecting individuals and families experiencing homelessness to appropriate housing and supportive services through the Coordinated Entry System (CES). This position serves as the first point of contact for participants, conducting assessments, verifying eligibility, maintaining accurate data in the Homeless Management Information System (HMIS), and collaborating with partner agencies to ensure equitable and efficient access to housing resources.
The ideal candidate is compassionate, organized, and skilled in working with vulnerable populations in a trauma-informed, person-centered manner.
Key Responsibilities
Participant Engagement & Assessment
- Serve as an initial point of contact for individuals and families seeking homeless services.
- Conduct intake and vulnerability assessments to determine eligibility and prioritize housing needs.
- Provide information, referrals, and warm hand-offs to emergency shelter, housing, health, and social service providers.
- Maintain a welcoming, non-judgmental, and trauma-informed approach in all interactions.
System Navigation & Case Coordination
- Assist participants in navigating the housing and services continuum, including completing applications, gathering documentation, and following up on referrals.
- Coordinate with internal staff and community partners to facilitate case conferencing, ensure timely placements, and reduce barriers to housing.
- Track participant progress and ensure continuity of care between service providers.
Data Management & Reporting
- Enter all assessments, referrals, and service updates accurately and promptly into HMIS or CES databases.
- Maintain up-to-date records and ensure compliance with confidentiality and data security standards.
- Participate in system-level reporting, performance monitoring, and quality assurance processes.
Community Collaboration
- Participate in case conferencing meetings and regional CES coordination activities.
- Foster strong relationships with community partners, housing providers, and service agencies to enhance collaboration and resource sharing.
- Represent the organization professionally in local homelessness coordination meetings and networks.
Qualifications
Education & Experience:
- Associate’s or Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred; equivalent experience considered.
- At least 2 years of experience working with individuals experiencing homelessness, behavioral health challenges, or other vulnerable populations.
- Experience with Coordinated Entry Systems and/or HMIS data entry strongly preferred.
Knowledge, Skills, & Abilities:
- Understanding of housing-first, harm reduction, and trauma-informed care principles.
- Strong interpersonal and communication skills with the ability to engage respectfully and effectively with people in crisis.
- Excellent organizational and documentation skills; attention to detail is essential.
- Proficiency with databases and basic computer applications (Microsoft Office, HMIS, etc.).
- Ability to work both independently and as part of a multidisciplinary team.
Working Conditions
- Primarily office-based with frequent community-based outreach and client interaction.
- May involve working in shelter or field environments and exposure to individuals in crisis situations.
Must maintain confidentiality and professionalism in all circumstances.
