We are seeking a compassionate and dedicated On-Call Caregiver to join our team at WI08, SYNERGY HomeCare of Madison in Monona, Wisconsin. As an On-Call Caregiver, you will provide personalized care and support to our clients in the comfort of their own homes. Responsibilities:
Provide one-on-one care and companionship to clients in their homes, focusing on their unique needs and abilities
Assist with daily living activities, including personal care, meal preparation, and light housekeeping
Encourage and support clients in their daily routines, promoting independence and autonomy
Maintain accurate and detailed records of client care, including progress notes and treatment plans
Collaborate with other caregivers and healthcare professionals to ensure seamless care coordination
Demonstrate a commitment to excellence, prioritizing client satisfaction and well-being above all else
Requirements:
High school diploma or equivalent required
Prior experience in the healthcare industry, preferably in a caregiving role
Ability to lift up to 25 pounds and perform physical demands of the job
Valid driver's license and reliable transportation
