Position Summary:
The Lead Caregiver supports clients, caregivers, and overall care quality by providing direct care, conducting home assessments, and assisting with field-based needs. This role also mentors caregivers, performs warm hand-offs, and ensures excellent service delivery.
Key Responsibilities:
Client Care & Home Support
Provide personal care, ADLs, companionship, and light housekeeping
Conduct home assessments and assist with care plans
Support new caregivers with warm hand-offs
Fill in for urgent or last-minute shifts as needed
Caregiver Support
Mentor caregivers and model high-quality care
Assist with care plan understanding and client transitions
Communicate field updates to the office team
Documentation & Communication
Document client status, changes, and incidents
Communicate client or caregiver concerns promptly
Collaborate with the Care Coordinator to support seamless care
Qualifications:
Home care or caregiving experience required
CNA preferred
Strong communication and relationship-building skills
Reliable transportation and valid driver’s license
Ability to work independently and support others in the field
