A closed-door Long-Term Care (LTC) pharmacy serving assisted living facilities in West Hollywood CA, is seeking an experienced Pharmacist to join our team. This role requires expertise in LTC pharmacy operations, and for the right candidate, there is an opportunity for a Pharmacist-in-Charge (PIC) position.

Key Responsibilities
- Oversee and manage the dispensing of medications for LTC patients in compliance with state and federal regulations.
- Ensure accuracy in prescription processing and medication management.
- Collaborate with healthcare providers and facility staff to optimize patient care.
- Maintain compliance with pharmacy policies and procedures.
- Supervise and support pharmacy technicians and staff.
- Assist in inventory management and quality assurance programs.
- Oversee and manage the dispensing of medications for LTC patients in compliance with state and federal regulations.
- Ensure accuracy in prescription processing and medication management.
- Collaborate with healthcare providers and facility staff to optimize patient care.
- Conduct medication reconciliation for patients transitioning between different levels of care, such as SNF and acute care settings.
- Maintain compliance with pharmacy policies and procedures.
- Supervise and support pharmacy technicians and staff.
- Assist in inventory management and quality assurance programs.
- Maintain accurate records of medication dispensing and patient interactions in compliance with legal and regulatory requirements.
- Pharmacist-in-Charge (PIC) Duties (if applicable):
- Ensure compliance with all pharmacy laws and regulations.
- Develop and implement pharmacy policies and procedures.
- Oversee staff training and performance evaluations.
- Manage audits, inspections, and licensing requirements.
- Serve as the primary contact for regulatory agencies and facility administrators.
- Pharmacist license in the state of California (active and in good standing).
- Experience in a closed-door LTC pharmacy is required.
- Proficiency with FrameworkLTC software is highly preferred.
- Strong understanding of regulations affecting LTC pharmacy operations.
- Ability to lead, manage, and optimize pharmacy workflow.
- Excellent communication and problem-solving skills.
