We are seeking an experienced Long-Term Care Pharmacy Consultant to support nursing facilities across Texas. In this role, you will ensure regulatory compliance, enhance medication safety, and optimize therapeutic outcomes for residents in long-term care settings. This position plays a key part in collaborating with interdisciplinary teams to promote best practices in medication management.
Key Responsibilities
- Perform monthly Medication Regimen Reviews (MRRs) in accordance with CMS guidelines (F756 & F757)
- Identify and report potential medication-related issues and recommend evidence-based interventions
- Maintain complete and accurate documentation per federal, state, and facility requirements
- Educate nursing staff and facility leadership on medication safety, appropriate drug use, and compliance standards
- Collaborate with physicians, nurses, and other healthcare professionals to improve pharmaceutical care
- Stay current on CMS regulations and long-term care pharmacy standards
- Active Pharmacist License in the State of Texas
- Minimum 2 years of experience in long-term care or geriatric pharmacy (preferred)
- Strong knowledge of CMS regulations and relevant F-tags (e.g., F329, F428/F756)
- Excellent communication, clinical judgment, and organizational skills
- Willingness to travel regularly to assigned facilities (mileage reimbursement or travel stipend provided)
