Sylva Valley Health & Rehab is hiring an Occupational Therapist (OT) and/or Occupational Therapy Assistant (OTA) to provide high-quality therapy services in a skilled nursing setting. Key responsibilities (OT)
-
Evaluate residents and develop individualized occupational therapy plan of care
-
Provide skilled OT interventions focused on ADLs, transfers, safety, cognition, and adaptive equipment

Document evaluations, progress notes, and outcomes to support skilled coverage and care needs

Coordinate with PT/SLP, nursing, providers, and the interdisciplinary team

Educate residents/families on safety strategies, home programs, and equipment

Participate in care conferences/IDT and discharge planning as needed
Key responsibilities (OTA)

Provide OT treatment interventions under the supervision of an OT

Document treatments and resident response/progress

Communicate changes in condition to the supervising OT and care team

Assist with resident/family education per plan of care

Support discharge planning and continuity of care
Requirements
- OT: Current Occupational Therapist license graduate of an accredited OT program
- OTA: Current Occupational Therapy Assistant license graduate of an accredited OTA program
- Strong documentation and communication skills
