Key responsibilities

Provide direct nursing care to residents according to physician orders and care plans

Administer medications and treatments safely and accurately

Observe and report changes in condition to the RN/provider as appropriate

Document care clearly and timely

Work closely with CNAs, nurses, providers, and the interdisciplinary team

Help maintain a safe, respectful, resident-centered environment
Requirements

Active Licensed Practical Nurse (LPN) license

Skilled nursing / long-term care experience preferred

Strong clinical skills, teamwork, and communication
