About the role
Sylva Valley Health & Rehab is hiring a Clinical Liaison RN/LPN to support admissions, referral development, census growth, and smooth transitions into our skilled nursing facility.
This is a true mix of clinical admissions review and relationship building. The right person will be comfortable reviewing referrals, communicating with hospitals and families, and representing Sylva Valley professionally in the community.
What you’ll do
- Build relationships with hospitals, case managers, discharge planners, providers, home health, hospice, and referral partners
- Review referrals for clinical appropriateness and communicate needs to facility leadership
- Help coordinate admissions and smooth transitions from hospital/community to facility
- Follow up on referrals, pending admissions, denials, and lost opportunities
- Use referral systems/platforms and EMR tools to support admissions workflow
- Partner with admissions, nursing, therapy, business office, and administration
- Track referral trends, barriers, and census growth opportunities
- Participate in evenings/weekends/on-call admissions support as needed

Requirements
- Active RN or LPN license
- Skilled nursing, long-term care, post-acute, hospital, admissions, or clinical liaison experience preferred
- Experience with referral platforms, hospital portals, EMR systems, or PointClickCare preferred
- Strong clinical judgment and ability to review referral information
- Excellent communication, follow-up, and relationship-building skills
- Comfortable with admissions urgency, evening/weekend/on-call needs, and census goals
- Professional appearance, customer-service mindset, and ability to work independently
