Core Responsibilities:
- Complete timely, accurate nutrition assessments, follow-ups, and documentation in accordance with CMS regulations and facility policies
- Develop and implement individualized nutrition care plans that align with physician orders and resident goals
- Monitor residents at nutritional risk, including weight changes, wounds, tube feeding, and altered intake
- Collaborate with nursing, therapy, food service team, and medical providers to ensure coordinated care
- Ensure diet orders are accurate, implemented correctly, and communicated effectively to the food service team
- Support survey readiness through compliant documentation and participation in audits as needed
- Provide education to residents, families, and staff as appropriate
- Participate in Quality Assurance and Performance Improvement (QAPI) activities
Qualifications:
- Must be a Registered Dietitian with CDR (Commission on Dietetic Registration)
- Must hold a state license in the state of which you are applying if indicated
- 1 Year experience preferred
Salary : $70,000 - $90,000
