We are seeking a compassionate and dedicated Direct Support Professional to provide high-quality care and assistance to individuals with developmental disabilities, aging-related conditions, or other health challenges. This role involves working closely with residents or clients to support their daily needs, monitor health status, and facilitate community engagement. Responsibilities
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts when necessary.
- Monitor and record vital signs, patient observations, and behavioral changes to ensure health stability and communicate findings to healthcare team members.
- Administer medications following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality.
- Prepare nutritious meals and assist with meal planning in accordance with dietary restrictions and individual preferences.
- Maintain a clean, safe, and organized environment by performing routine cleaning tasks and ensuring compliance with sanitation standards.
- Support residents in participating in community activities, social outings, and recreational programs to promote social integration and quality of life.
- Follow established care plans tailored to each individual’s needs, including dementia care, Alzheimer's care, autism support, or hospice care when applicable.
- Document all care activities accurately and promptly while respecting residents’ privacy rights under HIPAA guidelines.
Experience
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable.
- Demonstrated knowledge of patient monitoring techniques, medication administration, and vital signs measurement.
- Familiarity with specialized care such as dementia care, memory care, or hospice support is preferred.
- Experience assisting individuals with disabilities or developmental challenges using person-centered approaches.
- Knowledge of safety procedures including CPR and first aid certification is required; additional training in heavy lifting or the use of assistive devices like Hoyer lifts is advantageous.
- Strong background in caregiving roles involving long-term care or home & community care settings.
- Ability to work effectively within multidisciplinary teams including social workers, nurses, and other healthcare professionals while maintaining confidentiality under HIPAA regulations.
