The Sutter County Public Health Branch is looking for a Social Worker III to work in the Homeless Services Unit.

Under general direction, the Social Worker III – Health Branch provides social work services to clients in conjunction with the Public Health Nursing and or Homeless Services Staff; provides direct social services involving individualized treatment and specialized application of casework methods and skills; provides comprehensive casework services of a tangible nature; and performs other related work as assigned.

Class Characteristics
Social Worker III is the advanced journey level in the Social Worker series. Incumbents are normally assigned a selected caseload of the more difficult cases and develop coordinated treatment plans and/or case management plans. A Social Worker III may act as lead worker to other social service workers. Incumbents are expected to work independently. Social Worker III differs from Social Worker II in that the former receives the more difficult assignments requiring greater skill and depth of job knowledge in assessing problem situations and formulating plans for services. Incumbents normally work with a high degree of independence of action in administering services and in making use of agency or community resources Incumbents in the Social Worker III classification receive general direction from a Program Manager, Prevention Services Coordinator or other higher-level supervisor or manager, and may act as lead worker to a group of Social Workers, Health Program Specialists or a team comprised of other employees.

The eligible list established from this recruitment may be used to fill any future opening(s), including Part-Time and Extra Help, in this class for up to six months.

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other related duties may be required and assigned.
  1. Carries a caseload of difficult types of cases requiring a high degree of technical competence, such as situations where environmental, behavioral or medical issues affect personal or family life. 
  2. Performs client evaluations and assessments to determine social service needs of clients. 
  3. Develops and carries out social treatment plans for an assigned caseload.
  4. Refers clients to other staff members or community agencies.
  5. Makes a diagnosis of client problems and follows through with the social work treatment plans with a high degree of independence.
  6. Interprets policies, rules, and regulations to clients, their family members, and others.
  7. May act as a lead worker to a small group of social service workers or other employees.
  8. Assists clients and their family members in utilizing available resources for individual needs.
  9. Makes home visits in connection with casework assignments.
  10. Prepares and maintains case records, may perform data analysis, quality assurance or similar activities. 
  11. Participates in in-service training and other staff development activities to increase knowledge of the social work processes and achieve technical competence.
  12. Receives casework consultation from professionally trained staff members.
  13. Works cooperatively in a team setting. 
  14. Works with community organizations. 
  15. Makes referrals to outside resources.
  16. May provide Targeted Case Management or Enhanced Care Management services and engage in outreach and other activities to enhance services to Medi-Cal beneficiaries. 
  17. Some positions may include on-call or standby assignments, including evenings, weekends, and/or holidays. 
Knowledge of:  
Socio-economic conditions and trends; basic principles of individual and group behavior, social welfare concepts; effective interviewing and problem-solving methods, including motivational interviewing; public social service programs; community resources; current issues and practices in public social services.
  
Ability to:  
Communicate effectively in person, in writing and by phone in a clear and concise manner; analyze data and interpret directions, procedures and regulations; develop appropriate responses; perform job duties under stressful or emergency conditions; respond appropriately to situations; provide direct services to clients and their  families using a team approach with other professional public health staff members; understand and apply agency program, policy and procedures; obtain facts and recognize the relevance and significance; organize and maintain work details; establish and maintain effective working relationships with staff, clients, and outside organizations; apply laws, rules, and regulations to public health operations; explain public social service programs, policies, rules and regulations; interview, record and interpret case information; work within a community setting and effectively use appropriate resources and services; maintain confidentiality in accordance with legal standards and/or county regulations; communicate effectively both orally and in writing; operate a personal computer and other office equipment and related software; act appropriately in emergency and stressful situations; and operate a motor vehicle.

Education and Experience:  
Pattern 1: One (1) year of fulltime experience performing journey level case management as a Health Program Specialist II classification in Sutter County, or an equivalent classification within another agency*;
OR
Pattern 2: Equivalent to the completion of a bachelor’s degree in social work, Sociology, Public Health, Psychology, or related social or behavioral science, from an accredited college or university, and two (2) years of full-time social work *casework experience in a public or private social service, mental health, public health, educational or medically oriented agency.*

*Qualifying social work case management experience includes conducting comprehensive, strengths-based family assessments and evaluations using evidence informed tools to identify family resilience, risk factors and service needs, including families experiencing homelessness and housing instability. This experience encompasses managing a long-term, intensive caseload through regular home visits and sustained personal contact; developing and modifying individualized family service plans that are trauma-informed, culturally responsive, and reflective of each family’s goals; collaborating with cross-system partners such as housing agencies, behavioral health, and child welfare to link families to protective resources and community programs; and implementing service-level actions, safety planning, and program compliance measures within a relationship-based, family-centered framework.Requires the ability to think and act quickly in emergencies; effectively deal with personal danger; maintain mental capacity which allows the capability of exercising sound judgment and rational thinking under varied  circumstances; the ability to exert a small amount of physical effort in sedentary to light work involving moving from one area of the office to another; ability to see; ability to hear and communicate orally; requires sufficient hand/eye coordination to perform semi-skilled repetitive movements, such as using a computer, typing, data entry or use of other office equipment or supplies; ability to operate a motor vehicle; involves VDT exposure.

License or Certificate:
This classification may require a valid California driver’s license.

Disaster Service Workers: 
All Sutter County employees are designated as Disaster Service Workers during a proclaimed emergency and may be required to perform certain emergency services at the direction of the department and/or County.
