Supportive Care Specialist Group is seeking a compassionate Palliative Nurse Practitioner to join our growing clinical team serving primarily geriatric patients in hospital and LTAC settings. This role offers strong clinical autonomy, a supportive multidisciplinary environment, and the opportunity to make a meaningful impact on patients and families.

Responsibilities
- Daily rounding split between acute care hospital and LTAC
- Provide palliative care primarily for geriatric patients
- Support patients and families through complex care decisions
- Collaborate with physicians and interdisciplinary teams
- Maintain accurate clinical documentation
Qualifications
- Active Family Nurse Practitioner license (California)
- Minimum 2 years inpatient hospital experience 2 years primary care experience
- Strong communication skills and compassionate bedside manner
- Ability to work independently with a patient-centered approach
- Vietnamese speaking is a plus
Training & Support
- Structured onboarding and clinician shadowing
- Ongoing supervision from board-certified palliative physicians
- Support staff including social workers, chaplains, and office team assistance
