Overview
Supportive Care Specialist Group is seeking a compassionate and experienced Palliative Care Nurse Practitioner NP to provide patient-centered care in Southbay Area, CA across hospital and LTAC settings.
This role is ideal for clinicians who are comfortable with weekend coverage responsibilities while maintaining a structured weekday schedule. You will care for adult and geriatric patients with complex conditions and support patients and families through critical care decisions.
You will practice with strong clinical autonomy while working within a collaborative interdisciplinary team.
Compensation and Benefits
- $155000 plus base salary with bonus opportunities
- Monday to Friday daytime schedule with required weekend coverage

Key Responsibilities
- Conduct daily patient rounding in hospital and LTAC settings
- Provide palliative care consultations and symptom management
- Lead goals of care discussions with patients and families
- Collaborate with physicians nurses and interdisciplinary teams
- Develop patient centered care plans
- Maintain timely and accurate clinical documentation
Qualifications
Required
- Active Nurse Practitioner NP license in California
- Minimum 2 years inpatient hospital experience
- Minimum 2 years primary care experience
- Strong communication skills and compassionate bedside manner
Preferred
- Experience in palliative care hospice or geriatrics
- Bilingual Spanish speaking is a plus
