Key Responsibilities:
- Provide expert clinical care to patients with life-limiting illnesses
- Act as the point person for patients and families navigating hospice
- Manage medication and symptom relief, ensuring comfort and dignity
- Collaborate with social workers, chaplains, aides, and physicians
- Educate caregivers and guide them through emotional and logistical challenges
- Maintain up-to-date, accurate documentation
Qualifications:
- Active Minnesota RN license
- 2 years of hospice, oncology, or palliative care experience
- Passionate about patient-centered, compassionate end-of-life care
- Strong interpersonal, critical thinking, and decision-making abilities
- Commitment to ethical, regulatory-compliant practice
