What If You Could Get Ahead of the Next Crisis?
A lot of healthcare happens after something has already gone wrong.
The pain becomes unbearable. Breathing gets worse. A caregiver panics. Someone calls 911. The patient ends up back in the hospital.
As a nurse, you've probably spent plenty of your career responding to those moments.
Hospice Case Management gives you an opportunity to work differently.
At Superior Hospice, we're looking for an experienced RN who can look beyond today's visit and start thinking about what this patient and family may need tomorrow.
You'll manage a hospice caseload throughout the Northeastern Greater Twin Cities Metro Area, including North Branch, Wyoming, Lindstrom, Chisago City, Lino Lakes, Forest Lake, East Bethel, Cambridge, Isanti, and surrounding communities.
Your job isn't simply to react when something happens.
It's to anticipate what may happen next.
Good Hospice Nursing Is Proactive
You notice that pain is becoming harder to control.
You recognize a change in breathing.
You realize the spouse isn't comfortable administering the medication that's likely to be needed overnight.
You see that a patient's condition is progressing and the family isn't prepared for what they may see next.
Those are opportunities to intervene before fear becomes a crisis.
As the RN Case Manager, you're in a unique position because you know the patient, understand the plan of care, communicate with the provider, and coordinate the interdisciplinary team surrounding them.
You can connect the dots before they become problems.
Why Consider Superior Hospice?

What You'll Do

That includes:
- Manage an assigned hospice patient caseload

- Develop and update individualized plans of care
- Anticipate changing patient and caregiver needs
- Manage pain, dyspnea, anxiety, agitation, wounds, medications, and other symptoms

Your Previous Nursing Experience Matters Here
We're interested in experienced nurses from Hospice, Home Health, Oncology, Palliative Care, ICU, Critical Care, Emergency Nursing, Skilled Nursing, Long-Term Care, Assisted Living, Memory Care, Community Nursing, and Care Management.
Different backgrounds bring different strengths.
Maybe the ICU taught you to recognize subtle changes before a patient deteriorates.
Maybe Oncology taught you symptom management and difficult conversations.
Maybe Home Health taught you to assess independently without an entire hospital outside the door.
Maybe Assisted Living or Long-Term Care taught you what happens when a medically complex resident begins declining.
Those experiences can translate exceptionally well to hospice.
Hospice experience is preferred, but we're also looking for nurses with strong assessment skills, sound clinical judgment, organization, and the ability to think one step ahead.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license
- Strong clinical assessment and critical-thinking skills
- Ability to independently manage a patient caseload
- Ability to recognize and anticipate changing clinical needs
- Excellent organization and follow-through
- Strong patient and caregiver education skills
- Confidence communicating with physicians and providers
- Ability to work independently while collaborating with an interdisciplinary team
- Valid driver's license and reliable transportation
- Willingness to travel throughout the Northeastern Greater Twin Cities Metro Area

Don't Just Respond to What Happened. Help Prepare for What Comes Next.
Hospice nursing will always have unexpected situations.
But excellent case management can make many of those situations less frightening, less confusing, and better prepared for.
You'll know which symptoms need closer attention.
You'll educate the family before they're overwhelmed.
You'll communicate with the provider before symptoms become unmanageable.
You'll coordinate the resources your patient needs.
And when a patient's condition changes, the family won't be starting from zero.
They'll have a plan. And they'll have a nurse who knows them.
Superior Hospice is CHAP-accredited and backed by a Minnesota healthcare family with more than 30 years of experience serving patients and families.
If you're looking for $90,000 - $130,000, mileage reimbursement, community-based nursing, clinical independence, and the opportunity to practice more proactive nursing, we'd like to meet you.
