Job Summary:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. The LPN plays a critical role in delivering high-quality care to residents by supporting daily nursing operations, coordinating with interdisciplinary teams, and ensuring each resident's individualized care plan is followed with excellence and empathy.
Key Responsibilities:
- Provide direct nursing care to residents in accordance with their care plans
- Administer medications and treatments as prescribed by physicians
- Monitor residents’ conditions and report any changes to the RN or physician
- Document all nursing interventions and resident responses accurately and promptly
- Assist with assessments, admissions, discharges, and transfers of residents
- Collaborate with CNAs and other staff to ensure the highest quality of care
- Maintain a clean, safe, and supportive environment for residents
- Provide education to residents and families regarding health conditions and care needs
- Uphold all state, federal, and facility regulations and standards of care
- Participate in ongoing training and quality improvement initiatives
- Current and valid LPN license in the state of Minnesota.
- Graduation from an accredited practical nursing program
- CPR/BLS certification
- Long-term care or skilled nursing experience preferred
- Strong clinical, communication, and critical thinking skills
