- Supervising home health aide team members.
- Crisis intervention, grief support, and advance care planning.
- Acceptance of diverse backgrounds, flexibility, and strong interpersonal skills.
- Effective time management and attention to detail.
- Formulate the individualized plan of care and provide comprehensive nursing assessments to ensure the highest quality care.
- Provide timely communication and documentation of patient status and care interventions.
- Maintain positive relationships with community referral partners.
- Promote Lake Valley Hospice services with professionalism and care.
- Participate in the after-hours clinical on-call rotation.
- Valid driver’s license, auto insurance, and reliable transportation.
