Responsibilities
- Engage with community members in various settings during evenings and weekends to assess needs and provide support.
- Facilitate access to resources such as social services, healthcare, housing assistance, and other local programs.
- Build trusting relationships through active listening, empathetic communication, and culturally sensitive interactions.
- Collaborate with local organizations, service providers, and stakeholders to coordinate community outreach efforts.
- Document interactions accurately, including service provided, follow-up actions, and community feedback.
- Promote awareness of available programs through outreach events, informational sessions, and door-to-door engagement.
- Maintain professional boundaries while demonstrating a positive attitude that encourages community participation.
