Your Role

• Assess, monitor, and provide treatment for residents with wounds, pressure injuries, and skin integrity concerns

• Develop and implement individualized wound care plans in collaboration with physicians and the interdisciplinary team

• Document wound assessments, treatments, and resident progress accurately and timely

• Educate nursing staff, residents, and families on wound prevention and treatment strategies

• Ensure compliance with infection control standards and best practices in wound management

• Monitor residents for changes in condition and communicate findings appropriately

• Help promote quality outcomes and enhance residents' comfort and well-being

• Perform additional nursing duties as assigned to support resident care

Who We're Looking For

• Current Illinois RN or LPN license in good standing

• Experience in wound care or long-term care preferred, but not required

• Strong assessment, documentation, and communication skills

• Compassionate, dependable, and team-oriented with a resident-first mindset
