Job Summary:

Key Responsibilities:
- Identify ongoing needs and services of residents, promoting high-quality resident services.
- Evaluate and document changes in resident healthcare services and status.
- Participate in conducting resident evaluations and gathering information at move-in, 30 days post-move-in, semi-annually, and with any change in status.
- Collaborate with Resident Care Director (RCD) to promote infection control standards.
Qualifications:
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum one year experience in home health, assisted living, hospital, or long-term care environment.
- Excellent organizational and time management skills.
