Key Responsibilities
- Evaluate the needs of our members and recommend personalized care plans to achieve quality, cost-effective healthcare outcomes.
- Assist with developing ongoing long-term care plans and coordinate between members, families/caregivers, and care providers to ensure identified services are accessible.
- Provide resource support to members and their families/caregivers for various needs, such as employment, housing, and independent living.
- Monitor care plans, member status, and outcomes, providing recommendations to improve quality of care and service delivery.
- Interact with healthcare providers and partners to ensure member needs are met and collect, document, and maintain member information.
- Perform home and/or other site visits to assess member's needs and collaborate with healthcare providers and partners.
Requirements
- Bachelor's degree and 2 – 4 years of related experience or equivalent experience acquired through accomplishments and reflective of the level of this position.
