Schedule:

• Flexible shifts offered seven days a week

Key Responsibilities:

• Perform comprehensive annual health risk assessments in patients’ homes
• Review prior and current medical history, ongoing conditions, and medication use
• Offer education to patients regarding medical conditions and medication management
• Identify gaps in care and recommend solutions to enhance health outcomes
• No treatment planning or prescribing obligations—focus on thorough assessment and education

Qualifications:

• Current Nurse Practitioner license in Ohio
• Certification in Basic Life Support (BLS/CPR)
• Board Certification through AANP or ANCC
• At least six months of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
