Nurse Practitioner - Fairport, NY

Nursing degree, active Nurse Practitioner (NP) license and minimum 1 years Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered.

Providing specialized care for patients in nursing home and assisted living settings, this role supports comprehensive management of insurance members residing in skilled nursing and assisted living facilities. The focus is on treating patients in place when appropriate to reduce unnecessary hospitalizations and facilitate a smooth transition to home environments. Regular health assessments are conducted at least monthly to improve care quality and prevent emergency room visits or hospital admissions whenever possible.

Schedule/Hours

• Monday through Friday
• 8 AM to 5 PM
• No weekends or holidays

Key Responsibilities

• Develop individualized problem lists and medical treatment plans for residents
• Provide primary, preventive, urgent, and acute on-site care
• Identify and stratify high-risk patients for more frequent clinical evaluations
• Create contingency plans for managing high-risk conditions
• Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing
• Deliver hands-on bedside care, develop care plans, and manage orders
• Conduct single visits for patients transitioning from hospital discharge, assessing readiness for safe transitions out of skilled nursing facilities
• Manage panels of members across multiple nursing facilities, typically spending one day per facility, with flexibility to assess acute patients at alternate locations as needed

Qualifications

• Master of Science in Nursing (MSN) degree
• Current New York State Nurse Practitioner (NP) license
• Basic Life Support (BLS) certification
• DEA registration
• Experience in long-term care strongly preferred
