Deliver compassionate, patient-focused care as a Licensed Practical Nurse (LPN) in a home health setting, supporting individuals in maintaining independence and recovery within the comfort of their own homes. This full-time, contract opportunity involves managing a caseload of approximately 3 to 5 patient visits daily, utilizing your clinical expertise and strong communication skills to ensure high-quality care aligned with regulatory standards.
Key Responsibilities:
- Conduct skilled nursing assessments and implement personalized care plans for home health patients.
- Administer medications, perform wound care, and manage clinical interventions such as IV therapy as ordered.
- Monitor patient conditions, reporting any changes to the RN Case Manager or physician promptly.
- Provide education to patients and caregivers on disease management, medication adherence, and self-care techniques.
- Thoroughly document all nursing activities and communications using Home Care Home Base (HCHB) software.
- Collaborate with interdisciplinary healthcare teams to coordinate care and maintain compliance with regulations.
- Maintain infection control protocols and ensure patient safety during all visits.
- Participate in case conferences, quality initiatives, and ongoing training sessions.
Qualifications:
- Active, unrestricted LPN license in the state of practice.
- Minimum of 1 year experience in home health, hospice, or community-based care.
- Proficiency with Home Care Home Base (HCHB) electronic charting.
- Valid drivers license, reliable insured vehicle, and dependable transportation.
- Current BLS (Basic Life Support) certification.
- Strong clinical judgment, time management, and independent work capabilities.
Preferred:
- Experience with Medicare-certified home health agencies.
- In-depth knowledge of home health compliance and documentation standards.
- Excellent interpersonal skills with a patient-centered approach.
