What are the responsibilities and job description for the Sun House Case Manager position at Sun Community Service?
Provide on-site case management services to a maximum of 9 residents staying at Sun Community Service’s continuous stay shelter, Sun House. Provide community education regarding Sun Community Service’s role in Whatcom County as well as education around the homeless population and the challenges they face. Work closely with agency management and community partners to ensure coordinated program delivery.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
1. Using trauma informed and harm reduction best practices to provide case management services to individuals residing at Sun House including but not limited to: assessments, resources and referrals, safety planning, goal setting, housing search, budgeting, basic life skills, etc.
2. Collaborate with external agencies and community partners to provide a comprehensive support for residents staying at Sun House. Work closely with these partners to develop service and care plans. Attend or arrange client staffing as necessary to ensure residents are fully supported.
3. Provide full case management support and intake preparation for resident referrals who are not connected to external case management.
4. Maintain current knowledge and understanding of major systems that low-income and homeless individuals encounter and of resources they utilize in Whatcom County.
5. Maintain current, accurate and confidential records for tracking services, data management, and audit purposes. Create documents for resident support as appropriate.
6. Enforce policies, house rules, and Sun Community Service procedures to ensure that all residents, guests, and staff are safe and supported.
7. Review resident logs daily to ensure consistency and to identify possible areas of concern and to ensure up to date support for resident as well as respond accordingly per Sun Community Service policy and procedure.
8. Support residents in the transition to housing; work closely with the Sun Community Service PSH staff if a resident is an appropriate fit for that program.
9. In addition to case management notes, accurately document resident interactions in all-staff logs by following logging procedures.
10. Complete resident intakes for referrals including but not limited to: gathering appropriate documentation, reviewing referral packets, scheduling meetings, collaborating with external case managers and support teams to ensure a smooth transition.
11. Responsible for setting weekly meetings with each resident to check in on their goals and identify any potential issues or additional needs for supports.
OTHER RESPONSIBILITIES:
1. Attends regularly scheduled meetings and trainings
2. On-call shifts if appropriate
3. Other related duties and special projects as assigned

SKILLS AND ABILITIES REQUIRED:
1. Excellent communication skills; ability to interpret and/or discuss information with others which involves terminology, systems, or concepts which may not be familiar to many people.
2. Capability for independent, creative thinking in collaboration with others to address any issues.
3. Outstanding time–management and leadership skills.
4. Exceptional record keeping skills; ability to obtain and maintain accurate, detailed and confidential notes and client files specific to a variety of program and case management requirements.
5. The ability to function as a member of a team, working in collaboration with other staff members and in partnership with community organizations to build resources & services for clients.
6. Demonstrated organizational skills related to building program/group work/community.
MUST HAVES:
1. Bachelor’s degree in social work or a bachelor’s degree in a related field AND
2. Two years of relevant experience OR
3. Year for Year experience to replace degree.
4. Ability to travel within the agency’s service areas for community resource networking, workshops, meetings, and other job related professional activities.
NICE TO HAVES:
1. Experience working with individuals in the homeless community
2. Experience working with individuals with severe mental health diagnoses
3. Training in de-escalation, harm reduction, motivational interviewing and trauma informed care.
4. First Aid/CPR certification
WORKING CONDITIONS:
Work is generally performed in a fast-paced office environment with frequent interruptions and crisis situations. Majority of work centers on direct case management services and procedures that require a high degree of confidentiality, tactfulness, and respect for clients from a variety of economic, social and ethnic backgrounds.
