SummitWest Care is a nonprofit home health agency seeking Licensed Practical Nurses (LPNs) to support patient care within Mesa County.
This position offers flexible, self-managed scheduling and is ideal for LPNS looking for part-time or PRN work, with the opportunity to build a consistent caseload over time based on availability.
As an LPN in Home health, you will provide skilled nursing care in patients homes, working independently while staying closely connected with a collaborative clinical team. This role requires strong clinical judgement, adaptability, and clear communication, as each home environment and patient situation is unique.

What We're Looking For
- Clinicians currently licensed in the State of Colorado
- Current driver's license, reliable vehicle, and auto insurance
- IV Certification strongly preferred but not required
- Strong communication and commitment to accurate documentation
- Ability to work independently and manage time efficiently
- Professional, adaptable approach to home health environments
- Provide skilled nursing care in patient homes
- Assist with wound care, medications, and health monitoring
- Communicate patient status and changes or concerns promptly
- Document visits accurately and on time within our EMR
- Collaborate with RNs, therapists, and caregivers
- Support care plan goals and patient education
