As an LPN, you will work closely with residents, families, physicians, and the interdisciplinary team to provide exceptional nursing care in a skilled nursing and rehabilitation setting.
Responsibilities:
- Administer medications and treatments as ordered by physicians
- Monitor resident conditions and promptly report changes
- Complete accurate and timely documentation
- Assist with assessments and care planning
- Supervise and support CNAs and other nursing staff
- Communicate effectively with residents, families, physicians, and team members
- Ensure compliance with facility policies and state and federal regulations
- Promote a positive, resident-centered care environment
Qualifications:
- Current LPN license in the State of Colorado
- Current CPR certification preferred
- Skilled nursing or long-term care experience preferred, but not required
- Strong clinical, organizational, and communication skills
- Ability to work effectively as part of a team
- Compassionate and resident-focused approach to care
