• Location: Larimer County, CO
• Status: Full-time, 40 hrs./week, Hybrid (mostly remote, some office)

What You'll Do:

The position works closely with clinical teams to support prospective and current clients in navigating the SummitStone care system and to ensure clients are well connected to agency treatment providers, linked to community resources as needed, and have an established point-person within the organization to help guide their care and ensure timely and thorough follow-up regarding treatment options. The Treatment Navigator provides exceptional customer service while performing a broad range of clinical support duties. The position will require ongoing communication with clients and clinical staff via phone and email, scheduling clients for a wide array of services, the ability to effectively discuss agency paperwork requirements with clients, assisting clients in completing agency required paperwork in-person and through electronic platforms, medical record chart management, extensive knowledge of SummitStone service options, connecting individuals with community resources, and effective and ongoing collaboration with community partners to assist in care management efforts.

This position requires a high proficiency in computer skills; the ability to clearly and professionally document work, manage a high volume of emails, and communicate effectively with colleagues, consumers, and community partners. This position requires the staff person to have excellent organizational skills, to be detail oriented, and able to work independently and as part of a team. This position requires someone who is comfortable interfacing with clients accessing behavioral health services.

• Required: High school diploma; Proficiency with Microsoft Word and Excel; 1 years of administrative/clerical experience. 

• Preferred: Bachelor's degree; Bilingual/Bicultural

Some positions include driving a company vehicle and require a valid Colorado driver’s license.

Wage Ranges

The typical hiring range for this position is $20.40 to $24.48/hour or $42,432 - $50,918/year based on relevant years of experience.
