  • Support individuals in identifying strengths, preferences, and recovery goals.
  • Assist the crisis professional with development of safety plans and stabilization strategies when appropriate.

  • Provide follow-up contact and engagement as assigned to reinforce stabilization and connection to services.
  • Connect individuals and families to community resources including outpatient services, substance use treatment, housing supports, school-based services, and recovery programs.
  • Facilitate warm handoffs to ongoing providers and support attendance at initial appointments when appropriate.
  • Educate individuals and families about available behavioral health services and how to navigate the community mental health system.
  • Work collaboratively with clinicians, law enforcement, EMS, hospitals, and community partners during crisis response.
  • Complete required documentation for peer-delivered services and contacts in accordance with MDHHS, Medicaid, CMH, and CCBHC standards.
  • Maintain accurate and timely records of services and engagement activities within the electronic medical record.
  • Participate in required trainings including MDHHS-approved crisis training competencies such as trauma-informed care, de-escalation strategies, and harm reduction techniques.
  • Adhere to confidentiality requirements (HIPAA), ethical standards, and agency policies.
  • Participate in team meetings, supervision, and ongoing staff development activities.
