Stonebrook Post Acute is seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in patient care in SNF care. This role involves developing and implementing personalized care plans to improve patient mobility, function, and overall quality of life.
Duties
- Assess patients' physical conditions through comprehensive evaluations, including mobility, strength, and functional capacity.
- Develop and implement individualized care plans based on patient needs and treatment goals.
- Manage discharge planning to ensure smooth transition from hospital or clinic to home or community settings.
- Educate patients and their families on exercises, injury prevention, and health maintenance strategies.
- Document patient progress accurately using PCC and NetHealth and maintain detailed records of treatments provided.
- Stay current with advancements in physiology knowledge, anatomy, and medical terminology relevant to physical therapy practices.
- Participate in ongoing professional development to enhance skills in areas such as orthopedics, geriatrics, pediatrics, and specialized techniques like trigger point therapy.
Experience
- Proven experience working in SNF settings.
- Demonstrated ability to develop effective care plans for diverse populations such as pediatric, geriatric, orthopedic, or neurological patients.
