Full-time position, 40 hours per week
7:30 am - 4 pm Monday through Friday
Occasional weekends and call

JOB SUMMARY:

The case manager is a RN or LPN who collaborates with interdisciplinary team members, patients, and families to facilitate the transition of the patient from the hospital to a post-hospital setting. The case manager plays an integral part in the utilization and resource management process.

QUALIFICATIONS:

Licensed in the state of Oklahoma.
Minimum of 2 years clinical experience.
Case management experience preferred.
Strong skills in oral and written communications.
BSN preferred.

PHYSICAL REQUIREMENTS:

Able to speak clearly and distinctly with staff, physicians, patients, and families.
Able to extensively utilize telephone for communication.
Demonstrates adequate physical and emotional stamina to deal with stressful situations and to complete the necessary work schedule.
Able to sit/stand/walk for extended periods of time throughout the work schedule, with some light lifting and bending.
Demonstrates adequate vision, or correctable with glasses/contacts, for reading and preparing written documents.
Must have adequate perception of sounds or adequate hearing with corrections.
Able to hear spoken communications from staff, physicians, patients, and families as well as pages.
Demonstrates adequate manual dexterity to prepare written documents.
Able to work in a latex burdened environment.
