JOB SUMMARY:
This position will be half-time hospital clinical pharmacy and half-time clinic medication therapy management (MTM) through a new pharmacotherapy clinic (PTC).
Responsible for optimizing medication therapy for patients through medication management and collaboration with interdisciplinary teams. Promote and recommend safe, effective evidence-based use of medications while supporting patient outcomes, regulatory compliance and institutional goals. Provide clinical interventions and recommendations to providers. Monitor and adjust therapy for high-risk medications (e.g. vancomycin, aminoglycosides, anticoagulants, opioids) and make anti-microbial recommendations.
QUALIFICATIONS:

Four-year bachelor's degree or equivalent experience degree (B.S. or Pharm.D) from an accredited school of pharmacy

Current license to practice in Oklahoma

Maintenance of license through approved continuing education courses

Board of Pharmacy Specialties Certified preferred or at least one of the following: minimum of 3 years hospital clinical pharmacy experience or completion of PGY1 Pharmacy Practice Residency Program
PHYSICAL REQUIREMENTS:

Able to talk and converse distinctly with others in person and over the phone.

Able to walk/stand for extended periods, some lifting. Able to reach, pull, push, bend, stoop, and kneel.

Must have adequate perception of sounds or adequate hearing with corrections.

Demonstrates adequate vision, or correctable with glasses/contacts, to read/write, discern small objects, and to accurately fill orders.

Must have strength/stamina to deal with stressful situations.

Adequate fine motor skills and hand/eye coordination to type and accurately fill orders.
