Job Responsibilities
- Provide skilled PT services to adult and geriatric patients in their homes
- Complete evaluations and develop individualized care plans
- Deliver therapeutic interventions to improve mobility and safety
- Document services using EMR (WellSky/Kinnser preferred)
- Collaborate with nurses and therapy staff for coordinated care
Requirements
- Active Texas Physical Therapist License
- Current CPR Certification
- Reliable transportation, driver's license, and auto insurance
- Home health and EMR experience preferred (not required)
