Responsibilities:
- Coordinate and conduct MDS assessments, care plans, and quality measures for residents in our skilled nursing facility
- Ensure timely and accurate completion of MDS assessments and care plans, meeting established deadlines and quality standards
- Collaborate with interdisciplinary teams, including physicians, nurses, and therapists, to develop and implement individualized care plans
- Maintain accurate and detailed records of resident assessments, care plans, and quality measures
- Analyze data and identify trends to improve care processes and outcomes
- Provide education and training to staff on MDS and care planning processes
- Participate in quality improvement initiatives and quality management activities
- Ensure compliance with federal, state, and accrediting agency regulations and standards
- Minimum Requirements: Minimum 2 yrs of experience.
- RN Preferred-LPN encouraged to Apply
