Responsibilities
- Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to determine individualized care needs.
- Administer medications, injections, IV infusions, and perform wound suturing with precision and adherence to aseptic techniques.
- Provide specialized care for patients with complex conditions such as chronic illnesses, post-acute recovery, or developmental disabilities.
- Educate patients and their families on disease management, medication protocols, and health coaching to promote independence and wellness.
- Document all patient interactions accurately using electronic health record (EHR) systems ensuring compliance with HIPAA standards.
- Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to community resources.
- Manage emergency situations effectively by utilizing skills in airway management, acute pain management, and triage protocols during home visits.
Requirements
- Valid RN license in good standing with current state registration.
- Proven experience in diverse clinical environments such as hospital medicine, ICU, emergency room, or long-term care facilities.
- Familiarity with EMR/EHR systems for medical documentation.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-10), CPT coding, and laboratory procedures.
- Experience working with individuals with disabilities or developmental challenges is highly preferred.
- Ability to perform complex procedures such as catheterization, tube feeding management, phlebotomy, and sterile processing techniques.
- Excellent communication skills for patient education and interdisciplinary collaboration; ability to work independently while maintaining team connectivity.
