RN Case Manager Requirements:

RN Case Manager Responsibilities:
- Coordinate the overall care of the patient while interacting with patient, family, physicians, interdisciplinary team members and other healthcare professionals.
- Provide professional nursing care for both home health and hospice patients in their place of residence.
- Complete initial, comprehensive and ongoing assessments for patients to determine their needs.
- Conduct skilled nursing visits for pain and symptom management, wound care, medication administration, and other ordered treatments.
- Complete all documentation and charting accurately and in a timely manner.
- Monitor and supervise those involved in patient care to ensure the patient receives the best possible care.
Schedule:
- Full-Time: Monday- Friday
- Standard hours: 8:00 a.m. to 5:00 p.m., with flexibility based on patient needs
Pay:
- Based on experience and qualifications
Licenses Required:
- Active Utah RN license
- Current CPR certificate
- Valid Utah driver license with car insurance
