Essential Duties and Responsibilities
- Provide direct assistance to individuals with tasks related to daily living, such as bathing, dressing, grooming, toileting, eating, and mobility.
- Prepare meals and assist with meal planning according to dietary needs.
- Administer medication as prescribed by healthcare professionals and ensure individuals adhere to their medication schedules.
- Assist individuals in developing and enhancing their social, communication, cognitive, and motor skills through structured activities and interactions.
- Contribute to the development and implementation of individualized care plans based on the unique needs, preferences, and goals of each individual.
- Ensure the safety, security, and well-being of individuals by maintaining a clean, organized, and hazard-free environment and promptly addressing any concerns or emergencies.
- Maintain accurate and up-to-date documentation of services provided, observations made, progress achieved, incidents, and any significant changes in individuals' health or behavior.
- Collaborate with other professionals, including healthcare providers, therapists, case managers, and family members, to coordinate care, share information, and address the holistic needs of individuals.
- Advocate for the rights, choices, and independence of individuals with disabilities, and empower them to participate actively in decisions affecting their lives and community engagement.
- Willingness to work flexible hours to meet the participant’s needs.
- All other assigned duties
Administrative Functions:
- Monitor participant progress through case notes.
- Schedule transportation for participant appointments or community outings.
- Complete Special Incident Reports as needed.
- Keeps First Aid/CPR current.
- Participate in planning the monthly calendar and developing a new activity curriculum.
- Completes all other assigned paperwork.
- All other assigned duties.
Minimum Qualifications:
- Minimum of a high school diploma or GED (required)
- Two (2) years of full-time experience working with individuals with physical and/or developmental disabilities.
- Must be able to use all formats of mobile applications (Word, Excel, Safari, Zoom, FaceTime).
- Bilingual
- American Sign Language, Spanish, or Cantonese skills preferred. Some level of fluency for specific target populations may be required.
- Fluent report writing skills
- Able to recognize unusual or threatening conditions related to persons with disabilities
- Strong time management skills, patient, detail-oriented
- Proficient in Microsoft Office Suite

The noise level in the work environment is usually low to moderate.
Pay: $20.00 per hour
