Job Summary:
Join Stellar Health Group in Burlington, VT as an MDS Nurse! Responsibilities:
- Conduct and coordinate the development and completion of the resident assessment
- Conduct resident assessments in accordance with current rules, regulations, and guidelines
- Implement resident CAA's
- Participate in facility survey readiness and throughout the survey
- Work with our interdisciplinary care team to develop a comprehensive assessment and care plan for each resident
- Current unencumbered licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Vermont
- Two years of MDS and PPS experience
- Care Planning experience
