Physical Therapist Responsibilities
- Perform comprehensive physical therapy evaluations and assess residents' functional abilities and rehabilitation needs.
- Develop individualized treatment plans based on each resident's goals and clinical needs.
- Provide physical therapy interventions to improve strength, mobility, balance, coordination, and functional independence.
- Monitor resident progress and modify treatment plans as clinically appropriate.
- Provide education and guidance to residents, families, and caregivers.
- Collaborate with the interdisciplinary team to provide coordinated, high-quality care.
- Maintain accurate and timely documentation in accordance with facility policies and applicable regulations.
- Ensure all services are provided in accordance with the established plan of care and professional standards.
- Participate in care planning, team meetings, and other clinical activities as needed.
- Current and valid Physical Therapist license in the applicable state.
- Previous experience in a skilled nursing facility or long-term care setting preferred.
- New graduates are welcome to apply.
- Strong clinical, communication, and interpersonal skills.
- Ability to work effectively with residents with varying diagnoses, abilities, and needs.
- Strong organizational skills and attention to detail.

- $42–$55 per hour
