Responsibilities:
- Provide physical therapy services to the facility, promoting excellence in rehab care, interdisciplinary cooperation, and quality patient care
- Evaluate patients in a timely manner, as required by state, payer source, or facility requirements
- Screen patients upon admission and/or when requested by the director
- Develop an appropriate plan of treatment and goals for each patient
- Provide treatment as per the plan, developed after patient evaluation
- Communicate with other disciplines and interact with staff as necessary, based on the treatment plan
- Provide routine documentation according to Medicare, facility, and other payer source guidelines
- Participate in family conferences when requested
- Submit all required documentation and treatment charges in a timely manner, in accordance with policy and procedure
- Current MA State License as a Physical Therapist
- Must have flexibility to be a part of a dynamic and cooperative team, ensuring workload is distributed equitably across the department
- Capability to evaluate and assess patients, participate in consultations, teaching, and collaborate with residents, staff, families, and other professionals to:
- Plan individualized treatment plans and goals
- Discuss and plan appropriate discharge plans
- Teach and counsel patients and staff on best practices for recovery
