Licensed Practical Nurse (LPN)

Job Summary:

Berlin Health & Rehabilitation Center in Barre, Vermont is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality patient care to our residents, working closely with our interdisciplinary team to deliver exceptional care and support.

Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals
• Monitor and report patient progress and any concerns or issues that arise
• Maintain accurate and detailed records of patient care and treatment
• Participate in quality improvement initiatives to promote positive patient outcomes
• Demonstrate excellent communication and interpersonal skills when interacting with patients, family members, and healthcare team members
  
  
  

Requirements:

• Current LPN license in the state of Vermont
• High school diploma or equivalent required
• Ability to work flexible hours, including evenings and weekends, as needed
