ABOUT THE COMMUNITY SUPPORT PROGRAM OF JEFFERSON COUNTY Jefferson County Human Services developed the Community Support Program in 1996 to focus on helping consumers with a severe and persistent mental illness with various functional needs remain in the community and support them in their recovery journey to achieve their self-determined goals. CSP staff assist consumers in developing skills and working toward goals in education and employment, finding and maintaining stable housing, coaching them in activities of daily living, balancing their physical and mental health, developing and improving relationships, providing education to consumers and supports, and helping people to develop coping skills and strategies to manage mental health symptoms, as well as addressing challenges as they arise.
Position Summary
WHAT WE ARE LOOKING FOR In order to continue to serve all consumers that need services, the team is looking to hire a Community Support Professional II to join the team and provide symptom management, group activities, clinical assessment, treatment planning, and case management to consumers with severe and persistent mental illness of all ages. This position will also provide psychotherapy if qualified.
SOME ESSENTIAL RESPONSIBILITIES You will be successful in this role by completing the following tasks and responsibilities:
- Provides community-based symptom management services, facilitates individual and group activities, and monitors client
- Completes clinical assessments of client condition, ie: initial treatment plan; reviews; and Suicide Risk, AODA, Functional and Emergency mental health
- Develops treatment plans including quantifiable and measurable goal
- Provides case management services by utilizing community resources to coordinate care and treatment including crisis intervention, advocacy, and alternative housing/care
- Provides psychosocial rehabilitation skill development and coaching
Salary Information
The starting hourly wage range is $32.59 - $34.44 per hour depending on experience and qualifications. Earning potential up to $41.87/hour dependent on successful annual reviews.

Requirements:
- Master’s degree in social work or master’s degree in counseling and related Valid Driver’s license.
- Licensed Social Worker through the State of Substance Abuse Counseling preferred.
Preferred Requirements:
