- Some travel throughout surrounding regions may be required for this position.
- The candidate selected for this position will be given access to a state-owned vehicle and will not be required to use personal vehicles for travel.
- Due to this position requiring travel via motor vehicle, candidates will be required to have a valid Driver License.
- May require Utah licensure as a Social Service Worker.
- May perform on-call services under supervisor's direction, as needed.
Job Responsibilities:
Facilitate High Fidelity Wraparound Model services for families: utilize a strengths-based approach to safety planning, development of family team and family support systems, and wraparound planning for the purpose of maintaining children in their homes
Develop wraparound plan with families and agency partners with measurable outcomes and time bound specific objectives
Coordinate provider services and ensure the quality of services for youth and families
Prepare detailed case documentation including: opening and closing referral records and completing assessments, Strengths Needs & Cultural Discoveries, Wraparound Plans, contact notes, safety and crisis prevention plans, and correspondence, etc.
Provide regular communication and close collaboration with multiple community partners (e.g. Juvenile Court, Division of Juvenile Justice Services, Division of Services for People with Disabilities and Local Mental Health and Substance Use Disorder Treatment Facilities)
Ideal Candidate:
Bachelor's Degree in any relevant field or equivalent experience in any human services related field providing direct services to families
Experience implementing High Fidelity Wraparound Model
Active Social Service Worker license in the state of Utah
Is passionate about helping families receive the support and services they need.
Works with people in a manner which shows respect, sensitivity, tact, and professionalism
Loves interacting with community agencies.

Schedule:
- Monday through Friday; flexible four 10-hour days.

- May work with ages 0-21 and families.
- Risks found in potentially dangerous or unusual environmental stress situations, e.g., being subject to possible physical attack.
- Safety and risk issues inherit while working within a correctional facility with an inmate population are presumed.
- Work requires physical exertion. May require the ability to stand; walk over rough surfaces; bend, crouch, stoop, stretch, reach, lift moderately heavy items (up to 50 lbs.) in a recurring manner and/or for long periods of time.
Salary : $21 - $31
