Description of Work

This position supports North Carolina’s Money Follows the Person (MFP) program, which helps individuals transition from institutions back into the community. The Transition Services Coordinator serves as the central administrative and coordination support for transition activities across the state. The position works closely with the MFP team and Medicaid partners to track transition activity, ensure required documentation is complete, coordinate meetings and trainings, support outreach and stakeholder engagement, and assist with reporting and special projects. The role requires strong organization, communication, administrative and data tracking skills and involves regular interaction with internal staff, external partners, and families.

Knowledge Skills and Abilities/Management Preferences

Salary Grade Range: $39,244.00 – $68,679.00

Recruitment Range: $39,244.00 – $45,524.00

The Knowledge, Skills, and Abilities (KSAs)/ Management Preferences are not required. Applicants who possess the following skills are preferred:

  • Advanced Excel experience (tracking, reconciliation, reporting)

  • Experience coordinating meetings/trainings across multiple organizations

  • Experience working in Medicaid, disability services, or health care

  • Customer service skills
