The Long-Term Care programs at ALTSD administer home- and community-based programs to allow older adults to thrive in their homes and communities to reduce nursing home placements and emergency department visits.

At the direction of the Division/Program Director, the Social Services Coordinator for New MexiCare Program is responsible for:
New MexiCare program oversight and support to include eligibility determinations, financial assessments, and presenting on NewMexiCare programs.
Tracking and monitoring of participants and caregivers' cases, to include eligibility status, budget status, service planning, and follow up as needed.
Provide technical assistance and support to participants, caregivers, and contractors including resolving complex client issues.
Who are the customers?
ALTSD clients, other agency staff, contractors, home care agencies, and community organizations.

Minimum Qualification
High School diploma or equivalent and two (2) years of experience as a social work assistant and/or in social or community coordination, working with communities, working on health or social service related matters, social work/case management experience, and/or behavioral health care. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling two (2) years may substitute for the required experience.
Employment Requirements
Must possess and maintain a valid Driver's License. Background and business reference check and Driver's License verification are required, and employment is conditional pending results. Must possess and maintain a current Defensive Driving Course certificate from the State of New Mexico or must pass and receive a Defensive Driving Course certificate as a condition of continued employment.
Working Conditions
Work is performed in an office setting and community setting with extensive use of personal computer, telephone, fax, copier, and other related office equipment. Occasional after hours and occasional weekends may be required. Extensive travel throughout the state. This position requires travel and overnight stay.
