Why does the job exist?
This position will assist patients with discharge planning needs and provide clinical information to insurance carriers. The incumbent will provide utilization review concurrently with admissions to assure proper assignment of level of care and appropriate case management.
How does it get done?
This position will coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered. This position will facilitate swing bed multidisciplinary care plan meetings. Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options. Develop effective working relations and cooperate with medical team throughout the entire case management process and arrange abeyances with social services, health and governmental agencies. Record cases information, complete accurately all necessary forms and produce statistical reports and promote quality and cost-effective interventions and outcomes. Assess and address motivational and psychosocial issues while adhering to professional standards as outlined by protocols, rules, policies and regulations.

This position will work closely with all nursing departments as well as registration, physicians, families and insurance companies.

Minimum Qualification
Must possess a current license to practice as a Registered Nurse in New Mexico, or eligibility for such licensure in accordance with New Mexico requirements, or eligibility for a temporary license issued for employment at a specific institution in New Mexico and three (3) years of experience as it pertains to the essential duties and responsibilities of the classification.
Employment Requirements
Must successfully complete and pass a background check.
Working Conditions
Work is performed in an office and acute care facility with extensive computer and some phone usage. Some sitting, standing, walking, stooping, bending reaching and lifting up to twenty (20) pounds may be required.
