Osawatomie State Hospital is licensed by the State of Kansas to provide care and treatment for adults diagnosed with psychiatric disorders regardless of ability to pay. There are currently two hospitals on the Osawatomie campus in Miami county: Osawatomie State Hospital (OSH) and Adair Acute Care (AAC). Adair Acute Care are a 60-bed facility, certified by the Centers for Medicare and Medicaid Services.

The purpose of the Human Service Specialist – Case Manager is to assist the team social work specialist in completing discharge planning. The case manager will collaborate with community providers and the patient's significant others to locate resources for discharge. Case managers and social workers will work together as the primary liaisons to families as well as the community resources, relaying information to the team in written form as well as verbally. They will coordinate placement of patients to ensure that patients are released to the least restrictive environment possible, without endangering safety. Works independently in this position making many decisions on his/her own. Will also focus on building a robust benefits process to ensure the patient has the best outcome at discharge.
The essential functions of this position may require physical strength to frequently push, pull, twist, bend, squat, walk, stand for extended periods of time, lift and/or restrain residents with or without assistance and respond to aggressive resident behavior with specific, trained techniques. Be able to observe and respond to residents in distress.
Osawatomie State Hospital Adair Acute Care is funded in part by the Centers for Medicare and Medicaid Services and is subject to requirements bestowed by Federal and State levels of government conditions of participation.

Position Summary & Responsibilities
Position Summary:This position is an integral and critical part of the treatment and development of patient's discharge plan.Discharge planning begins at day one of hospitalization with the patient's goal being priority. The Case Manager is to assist the Social Work specialist in completing discharge planning. Additionally, Case Managers collaborate with community providers and the patient's significant others to locate resources for discharge.Social Workers and Case Managers will work together as the primary liaisons between families as well as the community resources, relaying information to the team in written form as well as verbally.Social Workers and Case Managers coordinate placement of patients to ensure that patients are released to the least restrictive environment possible without endangering safety. This incumbent works independently in this position, making many decisions on his/her own.
Job Responsibilities may include but are not limited to the following:
- Coordinates with assigned team Social Worker to develop plan for benefit needs for discharge planning.
- Consulting with the treatment team, mental health center liaisons, the patient, and involved family members to help determine needs.
- Exploring financial issues with patients to provide them with information to make informed decisions regarding their financial options.
- Utilizing resources creatively and interactions are flexible.
- Collaborates with assigned social worker to assess benefit needs prior to discharge and assist in establishing a plan to ensure benefits are completed in a timely manner.
- Documents in progress notes interactions with patients, community partners, and significant others regarding care and treatment.
- Provides Social Work coverage identified based on needs determined by the Director of Social Services and provides input during treatment team meetings, implementation, and progress regarding discharge planning.
Minimum Qualifications: Two years of experience interviewing, investigating, compiling information, documenting decisions, interpreting guidelines and/or providing technical assistance relevant to the agency's programs. Post secondary education may be substituted for experience as determined relevant by the agency.
