The Iowa Department of Health and Human Services (HHS), Division of Aging and Disability Services, Bureau of Community Integration, Targeted Case Management, is seeking a Disability Services Case Manager to join our team!
Disability Services Case Managers assist individuals with disabilities to gain access to resources they need to thrive. This position will be based in Des Moines and will be responsible for providing backup coverage for case managers serving multiple locations throughout Iowa.
Hours And Travel
Hours for this position vary based on the needs of the individuals we support, Monday – Friday. Travel is based on the needs of the individuals supported and in-home visits are required across multiple Iowa counties. Mileage is paid at $0.50/mile travelled according to the Department of Administrative Services (DAS) State Accounting Enterprise Procedures. There is very limited overnight travel.
Special Requirements
A valid driver’s license and a reliable vehicle are required for this position.
Core Job Duties
The Disability Services Case Manager works with individuals with Intellectual Disabilities (ID), Developmental Disabilities (DD), and individuals utilizing the Brain Injury (BI), Health and Disability (HD), Physical Disability (PD), Elderly and AIDS/HIV waivers. They help identify service needs, obtaining them, and evaluate progress in the provision of those services.

Graduation from an accredited four-year college or university;
OR
the equivalent of four years of full-time technical work experience involving direct contact with people in overcoming their social, economic, psychological, or health problems;
OR
an equivalent combination of education and experience substituting the equivalent of one year of full-time qualifying work experience for one year (thirty semester or equivalent hours) of the required education to a maximum substitution of four years.

- Develop individual care plans and service coordination across agencies.
- Assist individuals and/or guardians navigate the service system, secure crisis services, find new services and track outcomes.
- Ensure the individuals’ needs are met in a way that allows them to remain independent in their community and participate in decisions regarding their service plan.
- Previous experience as a case manager and a strong understanding of the disabilities listed above.
- Sound judgment and the ability to evaluate situations with accuracy and compassion.
- Excellent communications skills and the ability to modify communication methods to suit the individuals supported.
- Ability to correctly interpret laws, rules, regulations, policies, and procedures.
- Are experienced in using Microsoft Office programs and/or electronic medical records.
- Have interviewing and team building experience.
